Evaluations

If you participated in the Findlay workshop on August 27-28, 2003, please fill out and submit the Workshop and Facilitator evaluation forms to your left. Be sure to state clearly that the evaluations are for the Workshop.

The evaluation forms for the Course, Course Facilitator, and Course Learner should be filled out during Week 8 and 9, after you have completed all course assignments including the Action Plan.
